Phone Consultation
A no-pressure 20-minute call to discuss what's going on, answer your questions, and make sure I'm a good fit before you commit to anything. Completely free.
Initial Evaluation
Your first visit is a 60-75 minute, in-depth evaluation. I explore your history, goals, training, lifestyle, and the factors influencing your pain. Detailed movement analysis, hands-on examination, and a clear plan by the end of the session.
Follow-Up Treatment
Focused, intentional sessions that reassess, refine, and progress your plan. Manual therapy, strength training, movement retraining, and load management — always purposeful, always progressing toward independence.

Out-of-Network & Cash Pay
Foundation Physical Therapy is not contracted with any insurance company and is unenrolled with Medicare. Superbills are available for non-Medicare patients — you pay Sophia directly, then submit to your insurance for potential reimbursement. Call your insurer first to confirm they accept superbills.
Working outside of insurance means Sophia has full autonomy to treat you as an individual — not according to what an insurance company decides is "medically necessary."
Insurance reimbursement rates continue to decline while the cost of excellent care rises. This often forces clinics to shorten visits, rely on support staff, and juggle high patient volumes. That's not the care you deserve.
